Season Proposals
This form is for all of our Season Proposals, Spring Shows, Summer Musicals and Fall Shows. Our Season runs from September - August.​

Youth Proposals
This form is for our Summer Youth proposals. Youth Submissions can be considered at any time, but deadlines are different for Youth and Mainstage proposals.​

Advertisement Form
This form is for all Businesses and Patrons who would like to donate to the current or upcoming Season.

Costume, Prop and Set Rental
This form is for all Rental Inquires in the Geneva Theatre Guild Set, Costume and Prop Storage.

Photo Release Form
This form is to be used for any of our productions, in which pictures will be taken. Please submit these forms to the Producer at the beginning of the production, or directly after casting.
Invoice for Services Rendered
This form is to be used for any work that may have been done on any of our productions, if already approved by the Producer of that production. Please fill out this form and submit it to the Producer within 2 days of Opening.
Note: Payments will be handed out on the final performance date.
Reimbursement Request
This form is to be used for any payments, for any of our productions, made out of pocket, that have already been approved by the Producer of that Production. Please fill out this form and submit it to the Producer within 2 days of Opening.
